    Frequently asked questions

    Who can apply for a death certificate here?

    A spouse, child, parent, sibling, other relative, or a legal representative of the deceased. You will need a valid photo ID, and evidence of the death — or the body must pass through this hospital's mortuary.

    What documents are accepted as evidence?

    A medical report from the facility where the death occurred, a mortuary report, a police report, or a sworn affidavit — plus a copy of your own ID. Files must be PDF, JPG or PNG, up to 10 MB each.

    How much does it cost?

    There are two fees: a small non-refundable application (processing) fee paid at submission, and the certificate fee paid only after your application is approved. Both are shown before you pay.

    How do I receive the certificate?

    Once issued, you receive a secure, time-limited PDF download link by email. You can also collect a stamped physical copy at the medical records department with your photo ID and application reference.

    What if my application is rejected?

    You'll receive the reason by email. The application fee is non-refundable, but you will never be charged the certificate fee for a rejected application.
